Housing Services

CPC provides a variety of specialized housing options with case management support services for individuals who do not need 24-hour supervised care, but do require more support than can be found in regular low-income housing.  Accessible, affordable and safe community-based housing resources are provided according to client preference, eligibility requirements, and clinical need.  CPC has over 30 housing sites located throughout King County including apartments and community housing.  CPC is also able to provide a number of Shelter Plus Care and subsidized set-aside apartments and collaborates with many other housing providers to ensure access to specialized housing.

Clients are given the treatment they need, when they need it.  The treatment is based on a treatment and recovery plan developed with the client, family, other providers, and natural supports. It is the philosophy of CPC that client recovery is most attainable within the context of a community of care. Services emphasize client strengths and are based on an individualized and tailored care model.

Programs

Our housing services programs include:

Shelter Plus Care (SPC)
The SPC program provides a housing subsidy for our clients who are homeless but are able to live semi-independently with supportive services.
SPC is designed to link rental assistance to supportive services for homeless persons with disabilities and their families.  

Standard Supportive Housing (SSH)
The SSH program provides transition from supervised living services, housing stabilization, and case management services which address independent living, stabilization, and crisis management. This program provides treatment services which promote stabilization of mental illness symptoms and contribute to the individual’s success in community-based living.  Our recovery-oriented model emphasizes a partnership with the client to work toward their goals and develop social relationships and life roles.

Cluster Housing Program
The Cluster Housing Program provides a shared housing environment in which residents share living responsibilities with ongoing support from housing case managers. Services reduce symptoms, build resilience and support the client’s reintegration into the community.



Seeking Services

Whether you, your loved one, or an outside provider has referred you to CPC, your first step is to contact our front desk. An Administrative Service Representative can answer your questions and determine if our services are right for you, and provide you with our walk in hours.  The Administrative Service Representative will work with you to determine if we are able to meet your needs, or if we are unable to meet your needs, we will provide appropriate referrals.
