Community Housing Support Services
CPC’s Housing Support team provides services to clients living in CPC specialized housing. Services are designed to provide clients with the skills and support necessary to successfully live independently in the community and maintain their housing. The housing case management team provides:
- Case management
- Outreach and engagement, including meeting with the client in their home
- Support and assistance with activities of daily living and life skills (i.e. cleaning, cooking, laundry, grocery shopping, bill paying, etc)
- Regular house and/or apartment meetings to assist clients in resolving issues
- 24-hour housing crisis response
- Housing advocacy, including assistance working with landlords
- Vocational and educational support
- Referral and linkage to community services and coordination of services with non-CPC providers
- Assistance with transportation to agency and community appointments
Housing case management staff are located at our Wallingford location.
