Community Support Services
Individuals who suffer from chronic mental illness experience many barriers to recovery. These include housing instability, disenfranchised families, lack of socialization and independent living skills, and insufficient vocational skills.
Community Psychiatric Clinic provides Community Support Services to assist clients with chronic mental illness to live in the community as independently as possible, and to support their recovery from mental illness. Community Support Services staff work closely with the medical team to monitor client goals, treatment outcomes, and medication compliance by offering a range of recovery-oriented services including case management, supportive counseling, chemical dependency screening, housing support, pre-vocational services, peer and family support, and advocacy and referral.
Community Support Services are designed to serve clients who suffer from severe and persistent mental illness. The case manager and client work together to identify the strengths, skills and supports needed to promote recovery and ensure the client’s success in the community. Services emphasize assisting clients to manage their symptoms and maintain independence, stability, resiliency and well-being.
CPC also offers a distinct treatment program at our Northgate location for mentally ill adults who also have a developmental disability. CPC collaborates with the State’s Division of Developmental Disabilities to deliver appropriate recovery-oriented services and provide outreach and training to developmentally disabled clients in their own homes or residential facilities.
Community Support Services are available at our Northgate, Wallingford and Belltown locations.
