Taking the First Step
Whether you, your loved one, or an outside provider has referred you to us for services, the steps to follow are basically the same. The first step is to contact our Assessment Services department, if you haven’t already. Assessment Services provides a single point of access to the services available throughout CPC. The staff there:- screen calls and perform intake and assessments
- provide intervention services to individuals experiencing a mental health crisis
- are located at local and State hospitals to help coordinate services for clients being discharged into the community
- coordinate placement into supervised residential facilities
- assist clients who have limited resources in applying for public funding for services
- assist individuals in locating other referral sources available within the community.
During this phone call, you will be asked to provide information such as your name, address, contact information, date of birth, and social security number, as well as information about your mental health concerns; all of this information is kept confidential (see our Privacy Policy). Once we determine if our services are right for you, the phone screener will work with you to schedule an appointment for your face-to-face assessment.
Talk with a Mental Health Professional
An assessment appointment is where an experienced and caring Mental Health Professional meets with you to determine if the mental health concerns you are experiencing meet criteria for enrollment, if our services could be beneficial to you, and how we can work together to assist in your recovery. This appointment can take from 1 to 2 hours, depending on your personal situation, needs, and issues.
Getting Started
Depending on the results of your assessment, CPC will do one of two things: enroll you in services with us, or refer you elsewhere if another provider might meet your needs better. If we enroll you in services, we will assign you a counselor that will best fit your needs at the location that is most convenient for you.
Services
Your counselor will be your primary point of contact for all of the services you receive at CPC. It is his or her job to help you receive the services necessary to aid you in your recovery. CPC provides a wide range of recovery-oriented treatment options for individuals and/or families dealing with mental health and/or chemical dependency issues. Your individualized recovery plan could include any one or a combination of the following services:- Individual, family or group therapy
- Prescribing and monitoring medications
- Education on your disorder, reducing stress, resources available to you, and other subjects that might help you and/or your family manage and understand your illness
- Peer support, support groups and advocacy
- Case management and services coordination
- Referrals to other organizations in the community that might help you
- Housing or residential assistance
- Vocational services and support

“recovery-oriented,” meaning our goal is to help our clients overcome the symptoms and challenges of their mental health concern to lead a full and productive life. Each client is encouraged to find their own path to recovery, building upon their personal strengths and the support of their counselor and their peers to develop and achieve resiliency.
